Ⅰ. Personnel Files員工的人事資料
4. Accession Register入廠登記表
①.Contents of accession register include but not limit to: name, date of birth, address, ID number, signature, date of accession, photo, ID copy, contact phone number, resume, etc.
入廠登記表應(yīng)包括的內(nèi)容有:姓名,出生日期,地址,身份證號(hào)碼,員工簽名,入廠日期,相片,身份證復(fù)印件,聯(lián)系電話,工作簡(jiǎn)歷等等。
②.In time renew of expired ID.
若身份證的有效期已過(guò)的要及時(shí)更換。
③.Date of accession or date of form filling should not be any rest day on timecard.
要注意入廠登記表上的入廠日期或填表日期不能是工卡上的休息日。
④.Make sure that all candidates used their own genuine ID cards for accession registration.
要注意識(shí)別員工的身份證的真假,證件是不是其本人的。
⑤.Methods used in verifying ID cards: A. Log on Internet viahttp://www.lxsk.com/idcard.aspxor download Idcard.exe file viawww.u-jian.com, input the suspicious ID number and then they will tell you the result.

分辯身份證真假的方法有:a.資料網(wǎng)上復(fù)核:http://www.lxsk.com/idcard.aspx,輸入身份證號(hào)碼即可知真?zhèn)? b.參考這資料。
5. Labor Contract勞動(dòng)合同
①.Contracts should be signed immediately when employed and should be validated back within one month from the local labor bureau.
員工入廠后就應(yīng)簽訂勞動(dòng)合同,并在一個(gè)月內(nèi)到勞動(dòng)部門簽證回來(lái)。
②.Close attention should be paid to the signing date that must not be rest days on timecard; also, the validated date from local labor bureau MUST not be weekends or statutory holidays.
要注意入合同上的簽訂日期不能是工卡上的休息日,勞動(dòng)部門的簽證日期不能是星期六,日和法定假期.
③.Probation period in Labor Contract: No probation period for re-signed contracts; no longer than one month probation period for one-year-termed contract for fresh workers.
合同的試用期:老員工續(xù)簽合同不應(yīng)有試用期;新員工的合同期為一年的試用期不能超過(guò)一個(gè)月。
④.The minimum allowance for downtime should be in compliance with the local regulations and rules.
若合同內(nèi)容有關(guān)于停工待料補(bǔ)貼工資的問(wèn)題,則補(bǔ)貼工資的最低標(biāo)準(zhǔn)就是按當(dāng)?shù)刈畹凸べY標(biāo)準(zhǔn)。
⑤.A copy of signed and validated labor contract should be offered to signatories.
合同簽訂后應(yīng)該發(fā)一份副本給員工。最好還要求工人簽收確認(rèn)已拿到合同副本。
6. Juvenile Employee未成年工
①. Juvenile employee refers to laborers older than 16 but younger than 18.
未成年工是指已滿16周歲但未滿18周歲的勞動(dòng)者。
②. Special physical examination must be carried out for juvenile employees and the factory has to apply and register in the local labor bureau. Kindly find the attachedJuvenile Physical Examination FormandJuvenile Employee Registration Formfor reference.
工廠若有雇傭未成年工,則要為未成年工提供體檢并到勞動(dòng)部門申請(qǐng)登記,請(qǐng)參考未成年工體檢表和登記表,另還有申請(qǐng)雇傭未成年工表.
③. If applicable, it is best to hire adult employees. If there are juvenile employees in the factory, it will be better to let them be off when audited.
若可以,最好不雇傭未成年工。若有未成年工,最好不要讓他們參加驗(yàn)廠。
④. If juvenile employees are allowed to present at the audit, then please re-verify their ID cards.
若有未成年工參加驗(yàn)廠,則要特別注意驗(yàn)證他們的身份證真假和核對(duì)身份證是否是其本人。
Ⅱ. Wage & Time工資工時(shí)
1. Acceptable timing records可接受的考勤記錄:
①.IC swipe card timing records
IC卡刷卡考勤記錄。
②.Paper card timing records with signatures from employees monthly.
打卡鐘打卡考勤記錄,但需要員工每月在工卡上簽名確認(rèn)。
③. Manual timing records with accurate starting and ending time (including overtime recording), and signed by employees daily.
手工考勤記錄,但需要準(zhǔn)確記錄員工的上下班(包括加班)的時(shí)間,并由員工每天簽名確認(rèn)。
2. Notice in timecard preparing準(zhǔn)備工卡時(shí)注意:
①. There should be no 7 consecutive working day records
不能有連續(xù)7天上班記錄。
③.Total work days, total overtime hours and total overtime hours on rest days should be clearly stated in the timecard for easy checking.
把上班的總天數(shù),平時(shí)加班的總時(shí)數(shù),休息日加班的總時(shí)數(shù)清楚寫在工卡上,方便算工資和檢查。
③.The total overtime hours should not exceed 36 hours per month if the factory has no Overtime Waiver from local labor bureau; On condition that the factory has got the document from the labor bureau, the overtime hours can reach as much as 72 hours per month (Regular 8 hours plus 2 hours of overtime from Monday to Friday, 8 hours for Saturday and rest on Sunday).
若沒(méi)有向勞動(dòng)局申請(qǐng)延長(zhǎng)加班的批文,則加班時(shí)間每月不能超過(guò)36小時(shí),若申請(qǐng)加班批文后,每月加班 時(shí)間可以達(dá)到72小時(shí)(周一至周五,每天正常上班8小時(shí),加班2小時(shí),周六加班8小時(shí),周日休息).
④.There should be no united timing records on timecards, i.e., unvaried starting time and ending time for several consecutive days for the several employees.
不能有統(tǒng)一的打卡記錄,即不能有某些員工有連續(xù)幾天的上下班時(shí)間都是一樣的。
3. Payroll工資表
It is suggested that the payroll is calculated on the basis of timerate which is easier to be carried out, while piece-rate pay is troublesome, especially the overtime pay of piece-rate.
工資表最好是以計(jì)時(shí)工資為基礎(chǔ)來(lái)做,避免以計(jì)件工資來(lái)算,因?yàn)橛?jì)時(shí)工資簡(jiǎn)單容易做,而計(jì)件工資則復(fù)雜麻煩,特別是加班工資不好算。
①.The payroll form can be designed in accordance with practical situations, but overtime compensation should be separated from regular working time pay.
工資表按工廠實(shí)際情況設(shè)計(jì)表格,但一定要把正常上班工資與加班工資分列出來(lái)。
②.Close attention should be paid to deductions in payroll, if any. For example: each worker will be deducted RMB 100 per month as housing charge. Now supposed 8 workers share a room, then the factory will charge every room RMB 800 per month. If the local rent of room at the same level is more than RMB 800/month, then the deduction in this factory is reasonable; if the local rent of room at the same level is less than RMB 800/month, then the deduction is unreasonable, and the factory has to reduce the deduction until acceptable.
工資表內(nèi)若涉及扣款,則要注意所扣款數(shù)是否合理,例如:扣員工住宿費(fèi)每人每月100元,每個(gè)房間8人,則工廠每月每個(gè)房間就收員工800元的房租.若同一等級(jí)的房間在當(dāng)?shù)氐淖饨鸶哂?00元/月,則工廠的扣款是合理的;若同一等級(jí)的房間在當(dāng)?shù)氐淖饨鸬陀?00元,則工廠的扣款就不合理,那么工廠就要減少扣款數(shù)至合理。
③.There should be signatures on the payroll from workers.
工資表上要有員工本人簽名。
④.Date indicating wage paid should be clearly stated on the cover of payroll. Wage must be paid every month within 30 days. Example: Wage of July must be paid before August 30th.
工資表上要有發(fā)工資日期。工廠不能壓工人工資超過(guò)30天,例如8月30日前應(yīng)該發(fā)完7月份的工資。
Ⅲ. Social Insurance社會(huì)保險(xiǎn)
①.Insurance categories include: endowment insurance, occupational injury insurance, unemployment insurance, hospitalization insurance and maternity insurance.
保險(xiǎn)種類有養(yǎng)老,工傷,醫(yī)療,失業(yè)和生育保險(xiǎn)。
②.If the facility is incapable to buy all the five insurances for all employees (i.e., insufficient insurance categories or not all employees insured), then the factory must provide the following documents for verification:
A.Proof buying insurance for employees—after tax receipt or insurance-paid receipt from authorized bank.
B.Proved document offered by the local government saying that your factory has purchased insurance for employees as required. The document must clearly state that how many categories of insurances the factory has bought for how many employees and that this action has fully met the requirements from local government.
若不能為所有員工提供以上五種保險(xiǎn)(即缺少任何一種保險(xiǎn)或有員工未有買保險(xiǎn)),則要提供以下資料:
a.為員工買保險(xiǎn)的交費(fèi)證明—完稅證/銀行交費(fèi)單。
b.當(dāng)?shù)卣块T開具的證明,證明工廠已按當(dāng)?shù)氐囊鬄閱T工購(gòu)買保險(xiǎn),證明應(yīng)清楚說(shuō)明為多少員工和購(gòu)買了那些險(xiǎn)種,已符合當(dāng)?shù)卣囊蟆?/P>
Ⅳ. Factory Regulations工廠的規(guī)章制度
①.If any worker violates factory regulations, no fines should be made. It is best to give oral education, warning, etc.
若員工違反了工廠的規(guī)章制度,不能以罰款來(lái)處罰,最好是以口頭警告,記小過(guò),記大過(guò)來(lái)處罰。
②.No money or certificates should be required as “Deposits” by the factory.
不能收取“入廠押金”,也不能扣留或抵押?jiǎn)T工的證件。
③.Annual leave with pay is suggested to be stipulated in the factory regulations and be reflected in the payroll. 5 days for over one-year but under five-year workers; 7 days for over five-year but under ten-year workers; 10 days for over ten-year but under twenty-year workers and 14 days for over twenty-year workers.
廠規(guī)最好能提及帶薪年休假,工作滿一年未滿五年者5天,滿五年未滿十年者7天,滿十年未滿二十年者10天,滿二十年以上者14天,同時(shí)要在工資上反映出來(lái).
Ⅵ. Safety & Health消防安全與衛(wèi)生
1. Fire Drill消防演習(xí)
①.Fire drills in factory and dormitory must be carried out at least once a year, and records including words and photos must be kept.
要保留工廠消防演習(xí)記錄,宿舍消防演習(xí)記錄,最少每年一次,記錄應(yīng)包括文字記錄和相片。
②.Fire training records including training time, address, subject, participants, etc, must be kept orderly.
要保留消防培訓(xùn)記錄,記錄應(yīng)包括培訓(xùn)時(shí)間,地點(diǎn),培訓(xùn)內(nèi)容,參加人員等。
2. Fire Equipments消防設(shè)施
①.Fire equipments must be marked visible. For example: spraying red paint words of “滅火器” over the location placing fire extinguishers; or “消防栓” over the location placing fire hydrants.
消防器材應(yīng)有標(biāo)識(shí),如在滅火器或消防栓上方噴“滅火器”“消防栓”等紅色醒目大字。
②.There should be verified certificates from local fire services on fire extinguishers and fire hydrants. Generally, repairing proof or original certificate can serve the function. However, special attentions must be drawn to the period of validity that usually lasts for only one year. For hydrants, annual inspection certificates from local fire department will be required for verification in audit.
滅火器和消防栓應(yīng)有消防部門或具有同等資格的部門簽發(fā)的年審合格證,通常,滅火器上貼的維修合格證或出廠合格證就可以,但要注意其有效期是維修日期或出廠日期開始一年內(nèi)有效;消防栓則要有當(dāng)?shù)叵啦块T每年檢查的合格證明文件。
③.Hydrants must be covered by seals on which there are dates and chops from the factory.
消防栓要加封條,封條寫上日期加蓋工廠公章。
④.Periodical exams must be carried out to all fire extinguishers and fire hydrants in the factory. Records of these exams should be kept for verification. Please refer to the attached form.
滅火器和消防栓應(yīng)定期檢查,并做記錄,參考表格。
⑤.ABC type of fire extinguisher is recommended. Do not use AB or BC type of fire extinguishers only.
滅火器類型應(yīng)選用ABC型,不要選用單純的AB型或BC型。
⑥.Portable fire extinguishers cannot be placed directly on the floor. They should be hung up by hooks, placed in shelves or case with their bottom higher than 0.15 meter and their tops lower than 1.5 meter from the ground.
手提式滅火器不能直接放在地上,應(yīng)設(shè)置在掛鉤,拖架或滅火器箱內(nèi),其頂部離地面高度應(yīng)小于1.5米,其底部離地面高度不宜小于0.15米。一個(gè)滅火器配置場(chǎng)所內(nèi)的滅火器不應(yīng)少于2個(gè),每個(gè)設(shè)置點(diǎn)的滅火器不宜多于5個(gè)。
⑦.There should be a closed area with red lines painted on the floor placing fire hydrants or where fire extinguishers are hung up. The closed area must be kept visible and accessible.
消防栓或懸掛滅火器的地方,應(yīng)用紅色劃出一個(gè)隔離區(qū),隔離區(qū)內(nèi)嚴(yán)禁堆放物品以防阻塞消防通道。
3. Aisles, Exits & Emergency Lights疏散通道,安全出口標(biāo)志,應(yīng)急燈,
①.At least 2 exits per floor for both factory buildings and dormitory. Exits should not only be equipped with “Safety Exit” signs and emergency lights, but also be kept clear of blocks.
廠房,宿舍每層安全出口的數(shù)目不應(yīng)少于2個(gè),每個(gè)安全出口要保持暢通,應(yīng)有“安全出口”標(biāo)志和安裝應(yīng)急燈。
②.Emergency lights at every stairwell.
每層樓梯間應(yīng)安裝應(yīng)急燈。
③.On the wall of each floor must be posted evacuation plans on which fire extinguishers, fire hydrants, evacuation directions and viewer location must be clearly marked.
廠房,宿舍每層樓應(yīng)有清晰的疏散路線展示圖,圖上應(yīng)清楚標(biāo)明安全出口,滅火器,消防栓的位置,疏散方向和看圖者所在位置。
④.Aisles must be separated from production areas by yellow painted lines, and there should be evacuation arrowheads marked on the aisles.
生產(chǎn)車間應(yīng)以黃色通道線把生產(chǎn)區(qū)域與通道劃分出來(lái),通道上還應(yīng)標(biāo)示疏散方向箭頭。
4. Explosive-proof Lights防爆燈
①.Explosive-proof lights should be adopted in warehouses.
倉(cāng)庫(kù)照明應(yīng)使用防爆燈。
5. Protective Equipments防護(hù)裝置,防護(hù)用品
①.Protective covers to conveyer belts, open gears, grinding wheels, electric saws, conveyer engines, flywheels, etc. Take sartorius as an example, there should be protective covers to its upper and lower conveyer belts.傳動(dòng)帶,明齒輪,砂輪,電鋸,皮帶輪和飛輪等危險(xiǎn)部分都要安設(shè)防護(hù)裝置。例如縫紉機(jī)車需要安裝上下皮帶保護(hù)罩。
②.There should be metal gloves provided for electric scissors and electric saws operators. Refer toattachment. 電剪和電鋸的操作者應(yīng)配戴金屬手套作保護(hù)。
③.Earplugs must be provided for employees working under the conditions of 85dB.
在噪聲超過(guò)85dB的環(huán)境下工作的人員應(yīng)配戴耳塞。
④.Under any of the following circumstances, uniforms or aprons must be provided to workers. Other protective articles such as masks, gloves, hats, leg guards and shoe covers can be provided in consistence with actual needs.
A.During the operations there might be dangers like burn, scald or injures caused by machines;
B.Under working conditions of strong radiant or low temperature;
C.Under noxious, pungent, infective, and/or dusty working conditions;
D.Under corrosive, damp and/or dirty working conditions
在下列情況的一種,工廠應(yīng)供給工人工作服或圍裙,并根據(jù)需要分別供給工作帽,口罩,手套,護(hù)腿和鞋蓋等防護(hù)用品。
A.有灼傷,燙傷或者容易發(fā)生機(jī)械外傷等危險(xiǎn)的操作。
B.在強(qiáng)烈輻射熱或者低溫條件下等操作。
C.散發(fā)毒性,刺激性,感染性物質(zhì)或大量粉塵的操作。
D.經(jīng)常使衣服腐蝕,潮濕或者特別骯臟的操作。
⑤.Cotton masks, protective glasses and gas mask should be provided for laborers working in conditions of health-harming gas, steam or dust.
在有危害健康的氣體,蒸汽或者粉塵的場(chǎng)所操作的工人,工廠應(yīng)分別供給口罩,防護(hù)眼鏡和防毒面具。
⑥.Waterproof shoes must be offered to workers standing long on floors with water or other liquids.
經(jīng)常站在有水或者其他液體的地面上操作的工人,工廠應(yīng)供給防水鞋或防水靴等。
6. Broken Needle Records斷針記錄
①.Broken needles should be recorded in complete set. Miss of any part of broken needles will be considered a violation of social compliance in audit.
有使用針的工廠應(yīng)保留斷針記錄,要注意把整根斷針都保留并作記錄,缺少任何一部分都不行。
②.Records of broken needles should be updated every day!應(yīng)該每天記錄斷針情況。
7. Labels & Marks on Electric Switches電器開關(guān)的標(biāo)識(shí)
①.Every individual switch must be clearly labeled indicating its functions.
電器開關(guān)應(yīng)標(biāo)識(shí)清楚是控制什么電器的。
②.Words of “有電危險(xiǎn)” must be marked visible on electricity distribution box.
配電箱應(yīng)有明顯的警告標(biāo)志“有電危險(xiǎn)”。
8. First Aid Kit急救藥箱
①.At least one first aid kit per independent workshop or 100 workers. All kits must be kept unlocked and accessible.
每一個(gè)獨(dú)立的工作間或每100名工人至少設(shè)一個(gè)急救箱。急救箱不能上鎖,并容易找到。
②.Following leechdoms must be provided: alcohol and drugs with circles in the attached list.
急救箱應(yīng)常備以下藥品:酒精和清單內(nèi)打圈的藥品
③.No oral medicines should be found in the first aid kit.
注意急救箱內(nèi)不能有口服藥品。
④.There should be a drug list and drug-using records in the first aid kit.
急救箱應(yīng)有一份藥品清單和用藥記錄。
⑤.There are also first aid kits
宿舍也要設(shè)急救箱。
9. Toilets衛(wèi)生間
①. Toilet tissues, soap or hand wash should be provided.
工廠應(yīng)供應(yīng)衛(wèi)生紙,肥皂或洗手液。
②. Sufficient private room in the toilets (such as individual doors or separated walls)
衛(wèi)生間應(yīng)有足夠的私人空間(如設(shè)有間隔墻和門)。
③. Toilets should be separated by genders.
衛(wèi)生間應(yīng)分男女廁。(有圖案或者當(dāng)?shù)匚淖直硎?
10. Usage & Storage of Chemicals化學(xué)品的存放與使用
①. All chemicals used and stored should be labeled.
使用中或存放的化學(xué)品都應(yīng)貼有標(biāo)簽。
②. Second containers should be provided for chemicals used or stored.
使用中或存放的液體化學(xué)品都應(yīng)設(shè)有第二容器。
③. Corresponding MSDS should be posted on walls where chemicals are used or stored.
使用或存放化學(xué)品的地點(diǎn)都應(yīng)張貼相應(yīng)化學(xué)品中文的物料安全數(shù)據(jù)表(即MSDS)并上鎖.
